January 2025 brought the most significant Medicare drug benefit change in nearly two decades. The new $2,000 out-of-pocket cap on Part D prescription costs is transformative for people on expensive specialty medications. Here's every change and what it means in dollars.
The big one: Part D $2,000 out-of-pocket cap
Before 2025, Medicare Part D had no annual out-of-pocket cap. People with expensive medications — cancer drugs, biologics, specialty diabetes drugs — could spend $8,000–$15,000 per year in drug costs. Starting January 1, 2025, the annual OOP maximum for Part D is $2,000.

Medicare Prescription Payment Plan (M3P): Also new in 2025 — you can spread your drug costs over 12 monthly payments instead of paying large amounts early in the year when you hit your deductible and initial coverage phase. Opt in through your Part D plan.

IRMAA: if you earn more, you pay more
Higher-income Medicare beneficiaries pay Income-Related Monthly Adjustment Amount (IRMAA) surcharges on Part B and Part D. These are based on your income from 2 years ago (2023 income affects 2025 premiums).

If you had a one-time income event (Roth conversion, home sale, etc.) in 2023, you can file Form SSA-44 to request removal of IRMAA based on life-changing event.
Drug price negotiations: what's changing in 2026
The Inflation Reduction Act gave Medicare the right to directly negotiate drug prices for the first time. The first 10 negotiated drugs take effect January 2026:
- Eliquis (apixaban): negotiated price drops 56% for Medicare
- Jardiance: 66% reduction
- Xarelto: 62% reduction
- Januvia: 79% reduction
- Farxiga, Entresto, Enbrel, Imbruvica, Stelara, Fiasp/NovoLog insulin formulations
These prices apply to Medicare beneficiaries only. They do not affect private insurance costs.

- If you're on expensive specialty drugs, recalculate your annual drug budget — you may save $2,000–$8,000
- Review your Part D plan during the next Open Enrollment (Oct 15–Dec 7, 2025) — formularies and tiers change annually
